Who We Are

The Health Services Foundation (HSF) is a locally owned non-profit, 501(c)(3) organization established to encourage and promote health care facilities and related health care endeavors that serve the residents of Northwest Oklahoma. Our primary focus has been the inception and management of Northwest Oklahoma's #1 provider of Assisted Living, Providence Place.

Health Services Foundation was honored as a finalist for the 2011 Oklahoma Nonprofit Excellence award for Seniors

As we continue to maintain the highest level of care and exceptional quality in everything we do, we also focus on keeping our services affordable for our residents. We encourage our residents, families, staff and friends to include HSF/Providence Place among your charitable giving priorities.

Gifts made with cash or personal check can provide immediate help for programs, services and equipment.

These financial gifts provide a way to honor the memory of a loved one or to acknowledge someone special on an occasion such as a birthday or anniversary.

The Heath Services Foundation (HSF) is excited to facilitate the Roberta M. Tutt Scholarship Program for northwest Oklahoma residents who have a passion for nursing.  The family of Roberta M. Tutt funds this scholarship to honor the memory of Mrs. Tutt, who was a skilled, competent and beloved nurse to all. It is the Donor’s hope that the recipients of this Scholarship will help carry on Roberta Tutt’s love for nursing.

Eligible recipients of the Roberta M. Tutt Nursing Scholarship should be from or have a strong connection to northwest Oklahoma, specifically Woodward, Dewey, Ellis, Harper, Major or Woods counties.  Details of eligibility are included with the application. 

Applications are due by August 17, 2022 and recipients will be named by the middle of September.  The awards will be made directly to the accredited post-secondary educational institution upon confirmation of full-time enrollment in a nursing program.
