Levels of Care
Need-Based Levels of Care
Providence Place believes you should only pay for the services that you need. For this reason, we offer 5 levels of care.
We all age differently, so your level of care will be determined by a points-based assessment that captures only the services you need. The examples below will give you an idea of what your level of care may be.
- Level 1 - A resident who is level 1 is receiving only very basic services, the resident is mostly independent but may require assistance with reminders for meals, activities, and medication administration.
- Level 2 - A resident who is level 2 is receiving more services than level 1. At level 2, the resident remains independent in many activities of daily living; but requires regular additional services such as bathing, dressing, grooming, standby assistance, and more.
- Level 3 - A resident who is level 3 is receiving many services at regularly scheduled intervals, such as bathing, dressing, grooming, and assistance with transferring. A person who is level 3 is not independent in their activities of daily living.
- Levels 4 and 5 - Residents who are levels 4 and 5 need regular assistance beyond those services mentioned above.
